Yes. Rhadar supports fully customisable assessment forms.

Can clients complete forms themselves?

Yes, as long as they are with the Therapist and supervised they can complete the form. Alternatively the Therapist will complete the form on their behalf.

Do form results feed into reporting?

Yes. Data captured on forms flows into your operational and clinical reporting.
